Predictive Health Monitoring - Help

If you haven't registered to use Predictive Health Monitoring, you may want to learn more about it by clicking on "How do I use it?"
When you register, you will be asked to select a four digit PIN (Personal Identification Number), that will be known only to you, and you will be asked five questions, including: gender, Access Code, ZIP, country and birth date. For an Access Code, use the 3 to 5 character code provided by your Health Advisor, or simply enter PHM.
All information will be kept strictly confidential and the Predictive Health Monitoring system is HIPAA compliant. Simply put, we do not store any personal identifying information.
If you are registered, simply enter your ID and PIN in the boxes ^{provided and click on the} ^icon.
